* Water Readings: Ammonia, Nitrites, Nitrates, pH, Temperature. If you do not test please let us know.

* Tank Size

* Fish/es in Question

* Residents: what type of fish are in the tank in question.

* Physical Signs: how are the fish acting, swimming, bloated, not eating, marks on body and location, anything else you may or may not feel is important.

* Treatments: any treatments or meds already given to your fish.
